Questions to ask during the transition process.

As a participant transitions out of hospital, there are important questions that need to be asked to determine the care needs of the participant.

1. Does the participant have self-care problems?

2. Does the participant wish to, and capable of living alone?

3. Does the participant have family members or people they wish to reconnect with?

4. Has the participant been linked to community supports before admission?

5. What are psychosocial factors that will impact on the participants recovery?

6. Does the participant take three or more medications and have their medications

changed since the last admission?

7. Is the participant still at risk of/or suffering from a current mental health challenges?

The above questions will assist service providers in developing a strong care plan that will address the individual needs of the participant, and ensure a successful transition into the community.
